Maria
Executive Assistant
Maria joined Chicago Oculofacial Plastic Surgery in 2021, focusing on making the patient experience excellent from start to finish. She schedules patients for clinic and surgery and assists with financial and logistical questions. Maria keeps track of Dr. Phelps’s schedule and manages various business meetings and speaking engagements. She was born and raised in Chicagoland and is fluent in Greek.
Jazmin
Practice Manager
Jazmin joined Chicago Oculofacial Plastic Surgery in 2022, focusing on growing the practice and constantly improving the patient experience. She has experience working as a medical assistant in ophthalmology and neurology and providing various cosmetic treatments. She was born and raised in Chicagoland and is fluent in Spanish.
Hayley
Director of Nursing
Hayley is the director of nursing at Chicago Oculofacial Plastic Surgery. She serves as the surgical scrub and circulating nurse as well as delivering aesthetic treatments for our patients. Hayley comes to COPS with experience in the cardiac stepdown unit of a major Chicago hospital. She has a passion for aesthetics and when she’s not working she spends time with her husband and two dogs.
Grace
Registered Nurse
Grace joined Chicago Oculofacial Plastic Surgery in 2025 to provide aesthetic treatments and post-operative care. She earned her Bachelor of Science in Nursing from Saint Xavier University in 2020 and gained hands-on experience in plastic and reconstructive surgery at Loyola University Medical Center. Grace is excited to be part of Dr. Phelps’s team and looks forward to growing her expertise in medical aesthetics.
Sergio
Research Associate
Sergio is a medical doctor who fills many roles at COPS. He is the Research Associate for our ongoing clinical trials as well as quality control for medical records and documentation. Sergio is bilingual and plans to pursue residency and fellowship in the future.
Ashley, PharmD
Chief Product Officer
Ashley joined Chicago Oculofacial Plastic Surgery in 2025 as a skincare specialist at our Lincoln Park office. She earned her Doctor of Pharmacy (PharmD) degree and brings a strong background in product formulation, patient counseling, and dermatological care.
With a passion for evidence-based skincare, Ashley collaborates closely with our surgical and aesthetic teams to create customized regimens that align with each patient’s unique needs. She is committed to ensuring every product meets the highest standards of safety, efficacy, and satisfaction.
Janet
Medical Assistant
Janet joined Chicago Oculofacial Plastic Surgery to provide patient care and support throughout the treatment process. She completed her Medical Assistant externship program at Everest College in 2024, earning her certification as a Medical Assistant. Prior to joining the practice, Janet worked at Northwestern Medicine in the Ophthalmology Department, where she assisted the retina team with intravitreal injections and cared for patients with complex retinal conditions. She was drawn to aesthetics by her passion for helping patients feel confident and comfortable throughout their experience. Janet enjoys building meaningful connections with patients, fostering trust, and supporting them every step of the way. She is excited to be part of the ocular plastic team and looks forward to combining her ophthalmology experience with patient-centered care.
Gabriella
Esthetician & Patient Care Coordinator
Gabriella joined Chicago Oculofacial Plastic Surgery with a passion for aesthetics, skincare, and helping patients feel confident in their skin. She earned her Aesthetician license from Douglas J Aveda Institute in 2022, where she developed a strong foundation in skincare and personalized patient care.
Her interest in aesthetics began at a young age and has continued to grow through her education and hands-on experience. Gabriella is especially passionate about skincare and is always researching the latest products, treatments, techniques, and trends in the industry. She is committed to continuing her education and staying current with advancements in aesthetics to provide thoughtful, informed care tailored to each patient’s unique skin needs and goals.
Gabriella loves building meaningful relationships with her patients and creating an environment where they feel comfortable, cared for, and confident. She is excited to continue expanding her knowledge and skills while growing in the ever-evolving aesthetics industry.
